MediLink Consulting are recruiting Triage Nurses to work in an NHS hospital based Medway, Kent starting ASAP

Our client ideally requires 5 day cover between Monday – Friday 9am – 5pm or 8am – 4pm

They are looking to appoint a Registered General Nurse to work in their newly forming Health Hub.

The newly formed Health Hub will be managed by a GP, Trained Nurses and Administrators. Virtual consultations may evolve as the Health Hub develops and may involve face-to-face consultations in the future.

As a triage nurse for the Health Hub you will be responsible for the delivery of providing interventions for the patients to assess and treat conditions, manage long-term conditions as well as promote self-care. The post holder must demonstrate safe, clinical decision-making and expert care for patients as well as have a strong understanding of risk assessments. The post holder will work collaboratively with the PCN’s and be a key member of the wider primary care team.

Job Responsibilities:

  • Provide Nurse triage in the receipt and management of the E-Consult requests on behalf of the practices within the 2 PCNs involved in the E-Consult pilot.
  •  
  • Be a key player in their clinical team providing high-quality and timely patient centred care
  •  
  • In conjunction with the Health Hub GP, undertake assessments of a range of health and clinical needs of our patients using clinical practice skills and own clinical judgement to diagnose, treat, refer or discharge patients
  •  
  • Support and deliver 100% CQC clinical and operational compliance within the parameters of your role
  •  
  • Support the completion of clinical workflow laboratory reports, prescriptions and clinical documentation as advised
  • Work flexibly to support the needs of the Health Hub.


Experience / Qualifications: 6 months UK NHS experience required
