Description
MediLink Consulting are recruiting a Therapist to work in Shrewsbury, UK.

You will work within our client's Virtual Ward service, providing acute nursing intervention alongside the pre-existing UCR workforce.

The Virtual Ward is a system-wide initiative, working in partnership to provide patients with access to rapid assessment, treatment, virtual monitoring and care planning to stabilise and support an acute episode of ill health or a deteriorating chronic condition that would alternatively require admission to hospital.

The service offers an appropriate alternative to the admission pathway, which aims to deliver a recommended and agreed medical and nursing treatment plan. The focus is supporting the safe management of people in their own home.

The role and service offers integration and partnership working with team medical, nurses, social workers, AHPs and support workers, across the system to ensure a seamless service.
 
Experience / Qualifications: 6 months NHS experience
